Use this form if you wish to cancel a business registration with the Board. This form is for business that will no longer be operating in plumbing and/or gas fitting.
Individual Forms

Use this form for immediate submission of your new apprentice Applications and pay through our online portal. This can only be used by individuals wishing to obtain a new Apprentice Plumber/Gas Fitter registration who has not previously held a card with us. - Application for Active Status
Use this form if you have completed an approved Application for Inactive Status and you wish to re-activate your certification. - Application for Inactive Status
Use this form only if you hold a current certification, but are not actively engaged in the business of plumbing and/or gas fitting at this time and wish to have your card placed on inactive status. - Apprentice Registration
Use this form if you have never held a certification with this board and wish to become registered as an Apprentice. - Board Appearance Form
Complete this form if you wish to request for the Board to consider taking Journeyman exam prior to completion of the prescribed 2-year apprenticeship. You must be present for the Board meeting for the Board to consider your request. - Classification Declaration Form
Use this form if you are a Master or Journeyman Plumber to declare if you do or do not wish to obtain an Apprentice card. A minimum of an Apprentice card is required in order to perform gas fitting. Use this form if you are a Master or Journeyman Gas Fitter to declare if you do or do not wish to obtain an Apprentice card in order to perform plumbing. A minimum of an Apprentice card is required in order to perform plumbing. - Initial Application for Certificate – Journeyman
Complete this form upon passing a Journeyman exam and submit it along with required fees and documentation to activate your Journeyman certification. - Initial Application for Certificate – Master
